Fields not showing as expected

I have made a form here:

But it wont show the bottom sections of the form and I don't know why, there is nothing selected that would prevent this from showing up in the conditions that I can see.

Thanks for reaching out to Jotform Support. I cloned and tested your form to see if I can replicate the issue but everything seems to be working according to your conditions. The fields on the bottom end of your form are showing as expected. Check out the screencast below for my result:

There are other fields hidden conditionally such as those multiple choice related to PPE required but to what I see, they are working as expected with the conditions you've set. Can you indicate which fields you're having an issue with so we can make some tests and provide you with a solution? I've also cleared your form's cache which should help. Can you try testing your form again? If the same thing happens, let us know and we'll make some more tests to help you further.
