Potential Vendor Partnership

My name is Emily Evans, and I am with Clinical Trial Media. We specialize in patient retention and recruitment within the United States and globally.

We are in search of a vendor where we can create custom surveys. Capabilities we need would be having all questions appear on one page (with conditional logic implemented), and be able to include an image/video at the top of the survey. Zapier/One Drive integrations are also a must.

If you can confirm that JotForm exceeds all of the capabilities I have mentioned above, that would be great! From there, we can discuss scheduling a demo.

With Jotform, you can design surveys that display all questions on one page. Jotform also have a Conditions feature where you can add conditional logic to your form fields. You can also include images at the top of the form by adding your logo. Regarding with a video, you can use our widgets instead. Additionally, Jotform integrates seamlessly with both Zapier and OneDrive, allowing for enhanced functionality and data management.

Regarding with the demo, unfortunately, we don’t currently offer live phone support or meetings.
