How to link pre and post survey responses from the same individual?

Hello! I am looking for a survey platform that offers the capability to link pre and post survey responses from the same individual. Ideally allowing participants to enter a unique identifier during the pre-survey, and storing that so they will automatically be directed to their post survey when they enter it the second time, and both pre and post results will display as one response. Can any version of Jot Form do this? Thanks!
