Hidden fields are showing on my form

Hello,

I am having difficulty with a form that I have just updated. Despite having certain fields hidden, they appear before they are prompted to be shown. It only happens upon landing on the page when the option "Please Select" is showing. Once I click the other selections, the options show/hide as expected. When I click back to "Please Select", the hidden items also hide like they're suppose too. So, the problem of the shown (suppose to be hidden) fields only appear upon landing on the page. Weird. I can't seem to figure out why. I'm including the link below.

Thanks for reaching out to Jotform Support. I cloned and tested your form on my end, and I can see that there are hidden fields appearing even if you setup a condition. When I checked the conditions, it seems that they are conflicting with each other. You can remove the other states that will hide the hidden fields since they are hidden in the very beginning. Let me show you how:

1. In Form Builder, click on Settings on the orange navigation bar.
2. Then, click on Conditions on the left menu and open the condition.
3. Then, remove the field that you want to hide in the condition as shown below. Do this on other fields.
