Hi Nick,
Thanks for reaching out to Jotform Support. To add an image, you can use the Image element. Then, you create a Show/Hide Field conditional logic.
Adding Image:
- In Form Builder, click Add Elements on the left side of the page.
- Under the Basic tab, click on the Image element.
- Under the General tab, click on Choose a File.
- Click on the Upload File button under the Upload tab and select your image.
Creating a Show/Hide Field conditional logic:
- In Form Builder, click Settings in the orange navigation bar at the top of the page.
- Go to Conditions on the left menu.
- Click on Show/Hide Field.
- Create a condition for each image like in the screenshot below:
