Hi adfsasafs,
I have checked the form and you have integrated PayPal Checkout. Pay by check is not an option for this type of processor, so it's not possible to add this option to your form while they select the items from the product list.
The option is to clone the form and display the 2 forms by creating a Main form using the iFrame Embed widget, adding a Single Choice element, and applying the Conditions feature. In total, you will have 3 forms, take a look at the screenshot below:
First, let's clone the form and set that form as Pay by check. Let me show you how to set it up:
1. Login to your account and go to My Forms Page.
2. Select the form and click More at the right part of the page.
3. In the menu, select clone.
4. The clone form will open in the form builder. Scroll down, and select the My Product list.
5. Click on the Wand icon below the field to open Payment Settings.
6. Under the Payment Settings, click on the gear icon at the upper right part of the page.
7. The integration should have Purchase Order Integration. Purchase Order Tool allows you to create payment forms without payment vendor integration.
1. In Form Builder, click on the Add Element menu on the left side of the page.
2. Under the Basic tab, scroll down the list and choose the Single Choice field.
3. Click on the Gear icon on the right side of the field to open Single Choice Properties.
4. Under Single Choice Properties, enter the field label and then click Options to enter the selections.
6. Under the Widget tab, type iFrame Embed in the search field and click on it to add.
9. Under Properties, change the Align and Label Align field to Center.
10. Then change the length to 600.
Lastly, Let me show you how to create conditions:
1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Click on Conditions on the left side of the screen.
3. Under Conditions, click Add Condition and select Show/Hide Field.
