Dear Sir/Madam,
I am having a problem with my form (https://www.jotform.com/build/250768336918469). Many of the form fields are missing when in preview mode, which happened after I made some edits to the form.
I have tried several suggestions based on the previous post thread, including clearing up the cache and correcting the conditions. But, the problem still persists.
Hope you can help me with this issue.

Thanks for reaching out to Jotform Support. When I tested the live form, I don't see any missing fields. And base on how it looks now, it seems like you have already updated the form. Take a look at the screencast below:
Once we hear back from you, we'll be able to move forward with a solution.

The issue was fixed once I removed the form tab. I am not sure why.

When I cloned and tested your form with Form Tabs, everything worked the way it was supposed to. Take a look at the screencast below to see my results:
