    Hi vettold1,

I've cloned your form and copied the data on your spreadsheets, but I couldn't replicate the issue. I was able to pass the data of Lawn Size, Address and Zipcode. Check out my screencast below:

Spreadsheet to Form: Not all fields are being prepopulated Image 1 Screenshot 30 Can you try to remove the spreadsheets, then add it back? Let me show you how to do this:

    1. In Form Builder, click on the Wand icon of the Spreadsheet to Form widget.
    2. From the General tab, click on Remove file.
    3. Upload the Google Sheets again and select an Access code column.
    4. Click on Update at the bottom of the page. Spreadsheet to Form: Not all fields are being prepopulated Image 2 Screenshot 41

If the issue persists, can you share with us the spreadsheets you're using so we can try to replicate it on our cloned form?

Keep us updated and let us know how it goes.
