how do I change the last part of this to be a name rather than a number.

Hi lifecoachpetronya,
Thanks for reaching out to Jotform Support. You can only change the form URL, so it has a name in the place of the numbers, but it will also include your username. Let me show you how:
- Open your form in Form Builder, and in the orange navigation bar at the top of the page, click on Publish.
- Then, click on the Pencil icon to update the URL.
- Now, just click on Apply and you’re done.
Note that by changing the form URL, it will only let you change the number sequence after https://form.jotform.com/, that is, the form ID. You can't set the form URL in any other way, like a different website format or a title page.
If you want to use your own custom domain, you need to upgrade your account to Enterprise, as it is not available to regular Jotform accounts (i.e., Starter, Bronze, Silver, Gold).
