Hi ravi,
Thanks for reaching out to Jotform Support. To create a Request a Quote or Call for Price that integrates with your Magento Product Listing Page and captures product-specific requests from different customers. First, you will need to create a form that includes the basic fields that will capture the user's request. You can use Name, Email, Phone Number, Product Name and Long Text element. Let me show you how:
- In Form Builder, click on the +Add Element menu on the upper left side of the panel to open the Form Designer menu.
- Under the Basic tab, select the Name, Email, Phone Number, Short and Long Text element and add it to your form.
- Click on the X icon to close the Form Designer Menu, and Rename the Short and Long Text field.
Once you've completed the setup, you can generate a URL parameter to prefill the field in your form. Jotform enables you to generate prefilled URLs, allowing specific fields to be automatically populated with predefined values. You can use the URL format below:
https://form.jotform.com/YOURFORMID?productName=ProductXYZ
Also, make sure that the Unique Name of the field you want to prefill is correct. Let me show you how to get it:
- Click on the Gear icon of the Field to open its Field Properties window.
- Hover your mouse over and go to the Advance tab.
- Scroll down, click on the Field Details dropdown arrow and copy the Unique Name of the field.
You can now embed your form to your Magento website. Let me show you how:
- Click on the Publish tab on the orange navigation bar and select the Platforms tab on the left side menu.
- Under the Platforms menu, scroll down and select the Magento.
- Then, click on the Copy Code button and embed it to your Magento website.
If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?
Give it a try and let us know how it goes.

Hi Joeni,
Thanks for quick response. By reviewing your video, one has to manually input Product Name into the field. But we are looking to autopopulate Product Name into the field or capture product details.
Say for ex: I have 100s of products in my system
I want to create 1 common Jot Form which can be accessed by any shopper.
- I want to capture logged in shopper info.
- I want to also capture Product he is requesting.
When I get back to Jot Form reviews, I must see logged in shopper, Product info he browsed.
I don't want shopper to enter Product Name. -

Hi ravi,
I understand that creating the products one by one on each form is a bit of a hassle, Unfortunately, it's currently not possible to bulk import products into our Product List. hile the feature you’re looking for isn’t currently available, rest assured we’re always working to add new services and features to Jotform.
I've gone ahead and escalated your request to our Developer Team. When or if it's developed depends on their workload, how viable it is, and how many other users request it. If there are any updates, we’ll circle back to this thread and let you know.
Thanks for your patience and understanding; we appreciate it.
