Hi Kathy,
I believe the first thing you can do is turn the premade form into a Smart PDF form so that you can insert form submissions into it from another form. Jotform makes it easy to integrate your PDF files using Smart PDF Forms. Let me walk you through it:
1. On your My Workspace page, click on Create Form on the top-left side of the page.
2. Choose Import Form and then Import PDF Form.
3. Now, upload your PDF file.
4. I noticed that the original PDF contains two pages that display horizontally, and both have identical fields. To avoid having duplicate form fields in the Smart Form builder, we can just duplicate the mapping field and adjust it, as shown below.:
Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Once you have created your Smart PDF form, go back to this form, and download its form submissions as CSV. You can easily download your data from Jotform Tables . Let me show you how:
- On your My Tables page, hover over the form whose data you want to download, and click on View Table.
- On the upper-right side of the page, click the on Download All button.
- Now, select Download as CSV, Excel, or PDF to download your data.
1. Go to Import Data and Import Submissions Now at the bottom and authorize the app
2. In the Form Picker dialog, choose a form, then select Continue.
3. Apply the template format from the import tool to your Excel or CSV file. Follow this link to learn more.
4. Once your file is ready, click on Select Source File at the bottom and upload your XLS or CSV file.
5. Next, click on Import. That's it.
