Second page showing in the original PDF

When I preview my form, there is a second page. The original document does not have a second page. The fillable form looks great. I don't see anything in my settings.

Thanks for reaching out to Jotform Support. It seems that a blank page was uploaded together with your original PDF. Can you share a copy of the original PDF? You can upload it on Google Drive, and share the link with us.
