    Hello:

I plan to conduct a quality improvement project required for my Doctorate of Nurse Practice (DNP) curriculum at Maryville University.

I have 1 consent form, 2 assessment tools (each with 10 questions to answer yes or no) to be used once, and 2 evaluation tools (Likert sales: first one is 24 questions with answers 0-4; second one is 10 questions with answers 1-5) that will be used twice (pre and post-intervention). Given this, what are the options to collect and store the data using HIPAA-compliant processes? What are the costs?

I have 2 practice sites, one with 30 people and one with 50 people.

Unfortunately, there is no budget from my university for data collection, so this will be paid for by me. Given this, I am wondering if you have student rates.

    Thank you,

    Jill
