Form turned into Workflow-- Grayed out

Hello, we have a form that we use regularly TRANSMITTALS and all of a sudden it copied itself into a WORKFLOW that I can't click, but new submissions are going there instead of the original form. Can we get this back to normal?

I understand that you are seeing a grayed-out workflow icon on your Workspace dashboard for your form TRANSMITTALS. I want to clarify that the workflow icon you see on your Workspace dashboard is not a duplication. The icon you are seeing represents the workflow associated with your form. It is grayed out because the workflow can only be accessed when you open the form in the Form Builder page. If you want the icon to disappear in your Workspace dashboard, here's how:

1. In your My Workspace page, click Type from the upper-right.
2. Untick the Workflows checkbox to remove the icon in your Workspace.
