How can I Duplicate my jot form that gets filled out by my client and create a duplicate form that remove the phone number and email address from the duplicate form.

Hi marcmpf,
Thanks for reaching out to us for help. Our Import From a Web Page feature lets you easily clone your forms. Let me show you how to use it:
- On your My Forms page, tap on your form.
- In the popup list that comes up at the right part of the screen, tap on Edit Form.
- In the orange navigation bar at the top of the screen, tap on Publish.
- Tap on Quick Share and copy the Link.
- Back on your My Forms page, tap on the orange + Create icon on the upper left side of the screen, and tap on Form option.
- In the next window, tap on Import form.
- In the next window, tap on From a Web Page and paste in the link.
- Now, tap on Create Form and you're done.
You can delete the Data Field of your clone form by following this step:
- Open the newly created cloned form in Form Builder.
- Select the data field you want to delete.
- Click the Trash bin icon to remove the field.

Hi Marc,
Unfortunately, this can't be automated. But, you can also easily clone your existing form on your My Forms page. Let me show you how to do this:
- On your My Forms page, select the form that you want to clone or duplicate.
- Click on More on the right side of the page.
- From Form, select Clone.
- You will be redirected to the cloned form.
