How do i the below in cards format
This is the current one i have
I wanted to do something like the below.
1) Text is in the middle and the sublabels are inside the box just like below.
2) Postal code - is only half box and on the left (how can i do half only)

Hi Myorthoforms,
Thanks for reaching out to Jotform Support. I've checked your GUILDFORD ORTHODONTIC CENTRE ADULT INFORMATION FORM, and it appears that you're using the Multi-Line Question field for the Home Address. Check out my screenshot below:
I suggest using the Address field, instead. Let me show you how to add this:
- In Form Builder, click on Add Element on the left side of the page.
- From the Basic tab, select Address.
To make the sublabels inside the box, you can remove the default sublabels and add a placeholder. Let's do this:
- In Form Builder, click on the Gear icon of the Address field.
- From the General tab, remove the default sublabels.
- From the Advanced tab, enter the sublabels as the Placeholder.
