My clients are unable to submit their form after filling it out. And if they email it to me, it comes back blank.

Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
I also tested it on a mobile view and I was able to submit it on mobile view as well:
I have cleared your form caches as a precaution, test it again after clearing your browser cache. Also test it on a different browser or a device. If the issue persists, we'll do a more in-depth investigation and work on finding a solution.
Once we hear back from you, we'll be able to move forward with a solution.
