How to create a QR code that can print certificate?

    I’m trying to create a system where the student can scan the QR code, complete the heart saver pediatric skills sheet and then it’ll put the name automatically in certificate and email it to them.
    Should I use regular forms instead of smart forms?

    Hi crugher,

    Unfortunately, there is no option that allows users to use QR code that can automatically prefill data to the certificate and email to them is not available, while the feature you're looking for isn't available at Jotform right now, I've gone ahead and escalated your request to our developers. Exactly when or if it's developed depends on their workload, how viable it is, and how many other users also request it. If there are any updates, we’ll circle back to this thread and let you know.

    I’m not sure what you are talking about. It works for me
    When Form 1 is filled out, I want it to transfer the name to form 2.

    After watching a few of your videos several times, I was able to create proper link. However, the name and date is not centered in Form 2. Is there a way I can have the name centered in form 2?
