Hello! I apologize for the inconvenience however I was unable to solve the issue with the help documents and AI agent. We are an emergency animal hospital and are hoping to get a text alert or email when a client fills out certain aspects of our form. We are hoping to do this, so the form is flagged, or we are alerted to potentially infectious cases, so we know to isolate them and use caution. Any way of achieving this would be great.

Hello Katie,
Thank you for reaching out to Jotform Support. I didn’t fully understand the issue, do you mean you want to create an AI Agent that can collect forms and also trigger an alert via text or email when a certain form is submitted? If so, you can create an action for your AI Agent to fill up a form if a When Statement is met and Do and Action. Let me show you how:
- In Agent Builder, in the blue navigation bar at the top of the page, click on Train.
- Now, click on Add New Action.
- On the Action Settings, you can select the When Statement and choose Fill Form on the Do Statament.
Now for the Notification Email, you can set this up from your form. You can easily Set Up a Notification Email, so that you'll get an email after someone fills out your form. Let me show you how:
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. In the menu on the left side of the page, click on Emails.
3. Now, click on Add Email.
4. In the list that comes up, select Notification Email.
5. In the center of the next window, click on the Recipients tab.
6. Now, just enter your email address into the Recipient Email field, and click on Save. That's it, you're done.
