Hello,
I have just become aware that my Forms have disappeared. I am concerned I have been hacked. Possibly by an ex employee..

Hi Brandon,
Thanks for reaching out to Jotform Support. Can you share the Forms URL that you are unable to see in your account with us? We have a guide about Where to Find Your Form URL that you can check out. It might be that you deleted or archived the form by mistake, so consider checking the Trash and the Archive folder. Doing that is easy, let me show you how:
- In My Forms, in the column on the left side of the screen, scroll down to the Trash folder and click on it.
- If your forms are there, select them and then click on the Restore button at the top of the screen.
- Now, click on the Archive tab to access it.
- If your forms are there, select them and then click the More button at the top of the screen, in the horizontal bar.
- Finally, in the dropdown menu, click on the Unarchive option.
If your forms are not in the folders mentioned, we'll just need access to your account to do further check it. Don't worry, you can disable that again after we're done checking things out. Let me show you how to do it:
1. In the upper-right corner of your My Forms page, click on your Avatar/Profile Image icon.
2. In the small window that opens, click on Settings.
3. In the panel on the left, click on the Security tab.
4. Then, on the bottom-right side of the page, toggle Jotform Support Access to the On position.
Once done, let us know, and we'll be able to access your account to investigate better your issue. And can you let us know if you shared credentials with others, if yes, consider resetting your account password, doing that is also easy, let me walk you through it:
1. In My Forms, click on the Profile Picture in the top right corner, then in the dropdown menu, click on the Settings option.
2. In the sidebar on the left of the screen, click on the Account tab.
3. Now, click on Reset Password.
4. You'll receive an email with instructions on how to reset your password. Open the email and click on the Reset Password button.
5. Enter your new password, enter it again to verify it, and then click on Create a New Password.
There you go. Your new password is now set, and you can use it to log into your Jotform account. You can also check out our How to Reset Your Jotform Password guide for more information.

- Log in to your Jotform account and review the activity logs to see if there have been any unauthorized changes or deletions. You can find the activity logs under your account settings.
- Restore Deleted Forms (if applicable):
- If the forms were deleted, they might still be in your Trash folder. Go to the Trash section in your My Forms page and see if you can restore them.
- Change Your Password:
- To prevent further unauthorized access, immediately update your account password. Use a strong and unique password, and avoid sharing it with anyone.
- Enable Two-Factor Authentication (2FA):
- If available, enable 2FA for your Jotform account to add an extra layer of security.
- Contact Jotform Support:
- If you suspect your account was hacked or can’t find the missing forms, reach out to Jotform Support directly. Provide as many details as possible, such as the names of the missing forms and any suspicious activity you’ve noticed.
- Review Account Permissions:
- If your ex-employee had access to your account, check if their credentials are still active. If you use sub-user accounts, revoke their access.

Hi Brandon,
I checked your account, and I can only see one form. I also checked the deleted forms but there are none. Check out the image below:
This means that you only have one form on your account. Do you have another account you think that was created aside from this? If there's none, then it means that there are no forms missing.
