Document Upload, and maiden name

I have two questions, I need my high school comprehensive evaluation.

I passed 12th grade from India, there we don't have 4 year system, its the last year that gets the certificate. I have certificate with marksheet for the 12th year only. Will this be sufficient in order for you to provide comprehensive report?

second question is, my 12th grade was done before my marriage, so it has my name before marriage on it. But I am submitting request using name after marriage, so last name is now different? Do I need to submit anything else like marriage certificate in order to prove that this report belongs to me with my after marriage name?
