Chat disconnected, Account Limit: Form Fields Limit Met

    I want to keep the free account. It’s telling me that it’s not available because I’ve reached my limit and then I must upgrade, but I have not even received one submission.

    I was on the chat. She asked me to send her the error message and the chat disappeared and I’m unable to find another chat.

    Hi hermosaholistics,

    Thank you for reaching out to Jotform Support. The error that you are getting is wherein you have reached the maximum number of elements and/or widgets allowed on a form. The Starter has a limit of 100 fields to be used in the form. If you want to add more fields, then you will need to upgrade the account to the next plan level.

    Upon further checking, as well, it looks like the form is currently not available, and it is also currently locked due to HIPAA restrictions since you are collecting health information on your form.

    May I know if you received an email stating that you may be a HIPAA-covered entity? If so, if you are not a covered entity under HIPAA, kindly respond to the email by stating that you are not. This way, your forms are not going to be disabled. We need to ensure that the users are not collecting sensitive health information without being covered.

    Also, you will need to access your Data Settings and select No, I am not a HIPAA HIPAA-converted entity, so your forms will be reactivated. You will need to be logged in to your account. Let me show you how:

    1. In My Forms Dashboard, click on your Profile/Avatar in the upper right corner of the page.

    2. Click Settings from the menu.

    3. Select Data from the left-hand side of the screen.

    4. Click the Go Back text when the pop-up message shows up

    5. Finally, click on NO, I'm not a HIPAA-covered entity.
