How to create a Kish grid on the form?

I want to create a kish grid similar to this

to able to randomize who to interview within a household when going door-to-door. Its purpose is to eliminate bias

Thanks for reaching out to Jotform Support. You can use a Spreadsheet widget to create a Kish grid on your form. Let me show you how to do it:

1. In Form Builder, click on Add Element on the upper left side of the page.

2. Click on the Widgets tab.
3. Search for Spreadsheet.
4. Drag and drop a Spreadsheet widget on your form.

5. Click on the Wand icon of your Spreadsheet widget.
6. Enter your number of rows.
7. Enter your number of columns.
8. Enter the label for each column.

9. Enter the value for each cell inside the Default Values field.
