    Hi,

    Please help me retrieve my account, which has been suspended. Thank you for your prompt action.

    Hi Maria,

    Your account was suspended because you’ve got a form named W9 BLANK that was tagged as spam/phishing. We understand that this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.

    We did check your form, and it looks like the tagging was a false positive, so we've reactivated your account. You should be able to log in now.

    Hi Maria,

    Yes, since there is no issue with the form, you can still use it. I've also cleared your Form Caches to refresh your form, and it should be no issues.
