Fields in Notification Email does not match the form

There is a question on my jotform that others don't see. I had someone test the form and asked them to put something in every field - and their last question was my second to last question.

My form:

Hi Tara,

Are you perhaps referring to the form titled 'Self Evaluation for 2024. Accomplishments and Priorities'? If so, I checked the form and the notification email template does not contain the other questions. Did you perhaps add new fields to the form? You can go ahead and redo your Notification Email so the data in it corresponds to the current fields of your form. Here's how to do it:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.

2. In the menu on the left side of the page, click on Emails.

3. On the right side of the Notification field, click on the Trash Can icon, and delete the existing Notification Email.

Now that we've removed the old Notification Email, we'll just add a new one. Let's do it: 

1. While you're still under the Emails tab on the left side of the page in Form Builder, click on Add Email.

2. Next, select Notification Email.

3. Click on the Recipients tab and enter your email address in the Recipient Email field.

4. Now, just click on Save and you're done.

Try doing a test submission and you should be able to receive the complete data the form fillers provided.
