Conditions is not working properly

I have a form with conditional fields and i have them set to not be visible, but they still are. Can you help me with my issue? Probably something I did.

Thanks for reaching out to Jotform Support. I checked your form's condition and can see that it has a "Missing Fields". This is why the field that is supposed to be hidden is not working. Please note that once the condition is incomplete, it won't work. I would suggest to fix the missing fields in the condition and try again. If the issue persists, please tell us what is the conditional flow so we can have a better idea of how to fix this.

Thanks for your response! I fixed the missing fields. However, the fields
are still visible even though it says they’re hidden. I’m sure I’m still
missing something.

I checked your conditions and it is causing error on the logic as you're hiding the already hidden fields. You can check the updated condition below and copy it to your form. See the screenshots below:
