Request to reinstate suspended account

Hello, please reactivate our form, we have been using it for years now for our business, and we have done nothing wrong.

Your account was suspended because you’ve got a form named International Travel Application Form that was flagged as offering a fake service/job application scam. We understand this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.

For us to reactivate your account, we’ll need your permission to delete the offending form. You can create the form again after changing your account email address to a corporate email. 

Once we hear back from you, we'll be able to move forward with this.

I have received an update that we will also need the following information from you before we can reinstate your account:

1. Business Permit Registration 
2. Business e-mail address / website 
3. Any document to show that you are operating a legitimate service

Send all of these to reviewers@jotform.com with Thread# 19164901 as the subject, and we’ll let you know the status of your account after we’ve done a thorough review of them.
