Need Assistance with Form Submission

I had no problem uploading the requested documents to the form, but when I type in my contact information the cursor advances forward but the text does not appear.
The text field is white and I suspect that the font color is also white. Is there a way that I can change the font color?

Jotform is an Online Form Builder and doesn't have ddirectly manage the content or individual forms created by our users.  It seems you may have filled out a form managed by one of our users. If that’s the case, I recommend contacting the owner of the form for assistance with your request. You easily do this by replying to the confirmation email you received after submitting the form. Your reply will go directly to the form owner's email address or the designated recipient.
