Account Suspension--Need Reinstatement

    Hello,


    Our account was suspended for "fake services". My marketing coordinator, Sam Wray, is out of the country. Our account login is under her email sam91320@yahoo.com.


    We sponsored an event and hosted a drawing for a Wine Dinner from a local restaurant. We did not ask for any Personal Information and have used this jotform many times in the past.

    I checked your account, and it appears that your form Jennifer Tinsley State Farm - Event Form 2024 was flagged as offering fake services. If you believe that your account was suspended in error, we’ll need you to do the following:

    1. Send us the URL where your form is used.
    2. Send us a link to your website or proof of affiliation with a legitimate institution.
    3. Change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    I emailed the requested information. We are anxious to have access to the information collected at our event last Friday. Do you need additional information to unsuspend our account?
