Hi,
I have been having trouble trying to create a fillable form with you. I reached out a couple days ago and was given instructions to create a fillable form from scratch which i did and re-created the form 3 times; however, i am still not able to create a fillable form. Please advise.

Hi Lelauti,
Thanks for reaching out to Jotform Support. Are you referring to creating a Smart PDF Forms? If so, it is really easy to integrate your PDF files using Smart PDF Forms. Let me walk you through it:
1. On your My Forms page, click on Create Form on the top-left side of the page.
2. Choose Import Form and then Import PDF Form.
3. Now, upload your PDF file.
4. Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.
Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Check out the screencast below to see how to do that:
That's it. The final preview will look like the one below:
Give it a try and let us know if you need any help.

Hello Lelauti,
When you upload your PDF, Jotform Smart PDF Forms might not find all the fields automatically. If that happens, you can manually adjust the field connections. We have a full guide here on How to properly map your PDF form.
Give it a try and let us know how it goes.
