Thanks for contacting Jotform Support. We have a Revision History feature that lets you revert your form back to an older version. Let me show you how to use it:

1. At the top of the Form Builder page, click on the Timestamp under the title of the Form to open the Revision History.
2. Choose the version you want to restore the form to in the menu on the right side of the page.
3. Now, just click on Revert next to the version you want to revert it to. That’s it.

Give it a try and let us know if you need any help.

It seems you used the revision feature within the tables section. Are you able to try and use it within the form builder section instead? Please let us know if that allows you to use the revision history functionality correctly. Please also download all of your submission data prior to reverting to an older form.

Also can you kindly advise what changes that you made that you want reverted back exactly? And details of which version you wish to revert back to. I cannot identify it using your screenshot provided.
