I cloned a working form that is successfully integrated with Monday. I mirrored the field mapping, and this form is throwing an error.

It should function the same as this form (https://form.jotform.com/242425832979165), only the lead source has changed. This form is mapping correctly.
Please advise on the best solution.

Hi Clay,
Can you try removing the Monday integration and reintegrating it to the form? Let me show you how:
1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. In the menu on the left, click on Integrations and select the Monday.com integration.
3. In your Monday.com integration, click on the 3 dot icon and select Delete Action.
Give it a try and let us know how it goes.
