I have followed the instructions and have got both Name and Email fields on the form as shown. However, there seems to be a piece of missing information - we have the integration API for Mailchimp, but don't know where to put it when in Mailchimp?
When clicking Publish (next to Settings in Jotform), I can get the direct link to the form: https://form.jotformeu.com/8311XXXXXXXX1
So where does this go?
I can see html code for the form embed for a website - but we don't want to do that - we need to get the form in an email and send it out to people. Please can you give us some more info?

To send out the form to people over Email, get the form URL and share it over Email with a message of your choice. Note that users will have to click on the link to fill the form.
So, there's no way to actually embed the form into the body of a Mailchimp email then?

Unfortunately, that is not possible as such practice is not allowable anymore by the regulations in email sending standard and best practices. This is an internet-wide standard and such conformity is necessary for compliance by email service providers. And even if there are providers who would still allow usage of non-standard HTML tags, the other (most) email receiving servers will just reject the email or mark it as spam.
