Is there a way to find out who submits a jot form? I have a Review form. They can either select 1-5 poor to excellent. I want to be able to identify which customer is submitting the form so we can reach out and thank them. For those who select poor, we want to know how to do better.
Please advise.

Hi Ellie,
Thanks for reaching out to Jotform Support. You can add name and email fields to your form. This way, you'll be able to identify who submits the form. Let's do it:
- In the Form Builder, click on the Add Form Element and select the Name and Email fields.
We have notification email where in you'll get notified once a respondent submits a form. Data submitted will be included here. By default, this is added automatically, and the form owner is the usual recipient. Check out the screenshot below:
You can also view the data entries from the Jotform Tables. Let me show you how to do it:
- In the Form Builder, click on the app menu tab at the top of the page.
- Select Tables.

As you can see, the email field is here. You'll be able to contact the respondents personally.
